Building on 25+ years of interdisciplinary experience from within leading (business) companies (Mærsk Oil now Total Energies, Eurofins Danmark, DANDY, Fertin Pharma, Palsgaard Industri and DANAKLON) I have gained a significant knowledge and experience of interfaces within complex organisations.
The specific practical experiences are based on leadership positions within Engineering, Maintenance, Factory- and Operations management, HSEQ and from consultancy roles (external & internal) of Risk Management.
In combination with the role as Lead Investigator for more than 25+ major incidents of safety & financial impact, the experience attained within DEFRAGMENTOR is based on numerous experiences of how organsisations over time are struggling to remain proactive, as the portfolio of “perceived” critical safety work builds up.
Over time, a realisation of the importance of understanding “why things work” rather than “why they fail” have matured and became ingrained in my DNA. Observing a growing backlog of corrective work as normal in almost all organisations, I started to acknowledge that new ways of working and thus thinking was required in order to support any organisation to regain and maintain both proactiveness and resilience. On the basis of researching 50+ incidents i.e. why highly educated and trained specialists continued to fail addressing the future failures became paramount for my future work.
Conclusions of the review documented that the issue at hand did not relate to lacking competences. Instead, the basis of learning, risk assessments, development of procedures and education/training of employees, were limited by an outdated “single linear” concept, not applicable, towards the world of today of a more complex systemic character.
The conclusions could be addressed by four main elements:
- Designs were reused in new similar environments, accepted due to earlier successes
- A predominantly executional mindset with focus on efficiency
- Learnings were lacking proof of implementation i.e. controls were biased towards communication
- Priorities were in focus – backlogs were ignored and not subjected to re-evaluation of relevance.
The conclusions of the review were recognized to be similar the what I earlier have seen from other organisations in the past, without be able to clearly see the above.
The journey describe above later gave me the opportunity to put the knowledge into play and to transform the Risk Management system of a larger Oil & Gas Operator being hold hostage of earlier initiated work. After successfully defragmentation of ongoing activities and calibration of business categories , an initial 30% of perceived critical safety related work could be cancelled and eliminated. In continuation of keeping focus and apply the new systemic and mindset, the end level of cancellation of work was recorded at almost 65%.
The method applied is not limited to the Oil & Gas Industry and can be easily adapted by all types of business, interested in a transparent and decision management process, where critical work cannot be misplaced or hidden.
For inspiration, following scandals and incidents all had common findings of Early Warning Indicators beting ignored, neglected or misunderstood as relevant for intervention: Danske Bank, money laundering and incasso scandal, NASA Challenger explosion, Macondo Rig fire in the Mexican Rig.
The idea of the startup of DEFRAGMENTOR was later further developed and matured by working the elements of Risk Management from Entreprise Risks, Emergency Response, Crisis Management to Business Continuity.
